Your first 2 lines are filled in wrongly. Under name of offender, fill in the name of the player who broke the rule. Under offense, list down the rule broken/hacks used etc. As such, you should edit the first 2 lines to the following:

Name of offender: ItzVerY
Name of offense: Auto-Clicker

*Note: Give the full IGN in your reports next time. Video shows the IGN of the player to be ItzVerY instead of ItzVer.
