Well dying in the nether is something common, but dying in the safe zone nether plots is worst. People should really take care from what they are building because wither skeletons are escaping the grinder. If yours is making wither skeletons escape then please fix it. And if you found a plot which is letting wither skeletons escape talk to any of the staff members.
