I'm not sure why you might go to spawn or pvp when teleporting home, but the falling in the void I might be able to help with. This has happened ever since teleport met lag. One thing you can do to reduce it is to place a block at your /home. Stand on that block and /sethome again. Remove the block. Now your /home is one block in the air. This seems to help when teleporting at the same time the server lags. Also on survival (other servers don't seem to have this issue) I've fallen through one layer of dirt into the water when teleporting. The way I solved this was to stack up several solid blocks and set home on top of them. Instant fix, I never fell through again.
