»Factions PvP Tournament!

Well, as the title says, I will be hosting a PvP tournament on factions. There will be three different tournaments. Those are:
1v1
2v2
5v5 (if we have enough people wanting to participate)

The prize has yet to be determined, but I plan on hosting it approximately 10 days after reset.

The kit will be the same for all three tournaments. Regular protection iv unbreaking III, sharpness V and fire aspect II swords. Gapples will not be allowed, crapples will be allowed. Regen pots are also permitted, with the exception that no debuffs are allowed.

Now, If you would like to participate, here is the Format you must fill in. ( please follow this format it makes my life easier )
Note: Only the leader of the team must register for the team, I dont want all five or two members registering, only the leader. ( does not apply to solo's )
