    What launcher are you using? The default launcher from Mojang (premium only) has fixes for the bugs in Intel graphics chips.

    update your display driver
    if you have the option "chunk loading: multi-core" try toggling it to default to see if it still happens
    if these still dont work post a ss of your mods folder :v

    Probably RAM issue or Mc video settings. When you start your minecraft go to Task Manager . Go to Details and find javaw.exe / java.exe . Right click and set priority below normal .

    If this doesnt work delete .minecraft and reinstall.

    @Therez

    I have the same problem.

    Since survival resetted I keep crashing, I tried changing the launcher and it kinda worked.

    I get less crashes but there is still crashes but rarely happens.

    Maybe check if your Java version is up to date? Your PC looks old btw, how long has it been working for?

    First I would update java at https://java.com/en/download/manual.jsp if you haven't already done so.
    Next, open your minecraft launcher, click the edit profile button. Make sure that the Executable under Java Settings points to the javaw.exe file in the version you just installed. Also add the JVM argument -Xmx2G to allow minecraft to use up to 2GB of ram.

    If that doesn't work, completely delete Minecraft off your computer and re-install.

    Well since you are using optifine, you can also try disabling VBO (under video settings) and disable fast render (video settings -> performance), as those are some common errors which can cause crashes.
